Found problem, resolved it. The machine in question is an ancient k6/500 box that has been in service for well over 5 years at this point. The case sits on the floor with the cover off (who knows where it went :-). The problem was that the HSF was clogged with dust and the HSF/CPU interface had the thermal compound almost completely gone. In addition the clip holding the HSF to the CPU had gotten weak. Cleaned out the HSF, re-applied thermal compound to the HSF/CPU interface and bent the clip to provide increased pressure and the problem reported here went away and I have had no problems since.
