The mkinitrd script, version 4.2.0.3 is looking for /sbin/lvm.static which doesn't exist. It seems that lvm is already a static version. 

Simply replacing /sbin/lvm.static with /sbin/lvm in /sbin/mkinitrd worked for me.

I can confirm this is an issue...at least on amd64.  Additionally, the fix listed in the description works...i.e. replacing /sbin/lvm.static with /sbin/lvm in /sbin/mkinitrd.
