I don't use gnome, and so don't really want to install it just for this library. I removed the db and gnome dependancies in media-libs/gdk-pixbuf/gdk-pixbuf-0.22.0-r2.ebuild 
and it merged fine.
So shouldn't these then be optional based on the gnome use flag?

No , because other dependencies need "gdk-pixbuf with gnome enabled" and we cannot depend on an item with a certain USE flag set or not. 

Therefore it is hard enabled.
