After installing either xmms-1.2.10-r1 or -r2, xmms is displayed twice in the KDE Multimedia menu.

It looks like the xmms.desktop files from both /usr/share/applnk and /usr/share/application are causing this; removing the one from applnk solved the problem here under KDE-3.2.1

what about removing the one in /usr/share/applications?

did you have one in ~/.kde?
