Mplayer sees fit to set the PulseAudio output volume to 100%. For users on a Xonar Essence ST driving headphones directly, this can be *extremely* loud. I can think of other scenarios where this behaviour is at best irresponsible, and at worst dangerous to hearing.
Please see the attached URL for background information and a suggested patch, which may or may not have made it upstream by now.

Has this been fixed? Why is mplayer hard masked these days?

The patch from $url is at least in mplayer-1.0_rc4_p20100427.
