Did an emerge world and rebooted but no change. Masked 8.17 which downgraded me to sendmail-8.16.1-r2. During a previous update, cyrus-sasl-2.1.27-r6 had been upgraded to cyrus-sasl-2.1.28-r2. After downgrading sendmail, I re-emerged it and sasl and that fixed the issue. Sendmail and smtp auth can be fragile and I'm afraid to break my production instance again as I don't have time to troubleshoot it right now. I can work on it this weekend or try to set it up on one of my home servers. I know that's not very helpful from the bug submitter but I'm tied up with some family issues right now unfortunately. I wanted to go ahead and submit the bug report before too much time passed.
