After upgrading my kernel from 4.9 i experienced issue to connect with my sierra wireless modem. Some people have experienced same issue (gentoo forum). The issue was resolved with upgrading to >=ModemManager-1.6.10. I'll file a pull request on github for ModemManager-1.6.1{0,2} ebuilds.

Please try with the newer modemmanager pushed in tree. If this still does not work for you, feel free to re-open.
