/etc/modules.d is long deprecated. Please provide an equivalent
/etc/modprobe.d file instead.

It seems fixed to me:
$ emerge -pv slmodem

These are the packages that would be merged, in order:

$ ls /etc/modprobe.d/slmodem
/etc/modprobe.d/slmodem
