Updated from Gnome 2.12.2 to 2.12.3. All of a sudden I lost a few icons. Ex. Galeon fullscreen button, Evolution Calender and default folder button, etc.. Finally found out if I go back to hicolor-icon-theme-0.8 they come back.
Hmm.. See http://bugzilla.gnome.org/show_bug.cgi?id=315841 I am a little confused. The latest gnome-icon-theme throws a bunch in hicolor/stock. Should it not?
Same append here. Downgrade to hicolor-icons version 0.8 and everything is back again
Here is a patch and updated hicolor ebuild. This adds back in stock/generic and stock/document entries into hicolor/index.theme. I think they were left out on accident. I am using the gnome bug I posted earlier as the upstream bug. I don't think freedesktop bugzilla is working for icon-theme yet.

Same thing happened here too, with Xfce4 (SVN).. downgrading hicolor-icon-theme to 0.8 fixed it.

Applied the patch as that seems to be a mistake from upstream. Please sync and test in ~2 hours. Sorry about the delay.
