Unfortunately this is a known issue in kaffeine, not Gentoo-specific. Not using --with-xorg prevents the crash of kaffeine running standalone, but it has no effects on kaffeine embedded in konqueror. Upstream comments look like these troubles are widely unkown. I already filed a bug, but they couldn't help so far - so may be it's gentoo specific, but as you think neither, so ...
