Someone should quantify the problem better than I can, but the problem is in essence that emerge ignores a binpkg without mentioning it, if the reason is a mismatched dependency. In my case, the binpkg firefox was built with icu-74, and the target machine had icu-75. `emerge -k firefox` would ignore the binpkg without emitting a message.
