Kmail is missing a number of plugins, specifically the "external editor" plugin.  It seems that there are no USE flags to add plugins.  It is possible that they should be built into Kmail by default.

It looks like this is my mistake.  These plugins are installed via in the kde-apps/kdepim-addons package.  Please close this bug!  Sorry about that.
