When emerge x11-base/xorg-x11-6.8.0-r4 it fail and result me :
* Failed Patch: 5160_all_6.8.0-radeon-fix-monitor-detection.patch!

Now im using xorg-x11-6.8.0-r2

I can't reproduce this with your USE flags:

 *   5150_all_4.3.0-ia64-radeon-preint10.patch ...                        [ ok ]
 *   5160_all_6.8.0-radeon-fix-monitor-detection.patch ...                [ ok ]
 *   5200_all_6.8.0-newport-accel-v4.patch ...                            [ ok ]

Do you have anything for xorg-x11 in /etc/portage/package.use?

Also:
cat /var/db/pkg/sys-devel/patch-[0-9]*/CFLAGS
Rebuild patch with very tame CFLAGS -- perhaps "-march=pentium4 -O2 -pipe" or even "-march=i686 -pipe."

I solved the problem re emerging "patch" it was compiled with different CFLAG
