Totem 0.100 have a nice feature that allows you to build it without needing gnome. This can be achieved by using the "--enable-gtk" flag.
I think someone can edit the latest ebuild to add a -gnome USE flag.

It seems that the gtk only option is a little bit idiot, because anyways you need gconf, libglade, gnomevfs, and so. I'll try to investigate some more.

Speaked in bugzilla gnome, and said to me that there are poor diferences when using this flag. So forgive it.

It doesn't build anyway gtk-only (never has in the tarballs since it got introduced), when it gets fixed up it will be included.
