I need to know what version of MPlayer you are running first...
Bug happens with both mplayer 1.0_pre4-r7 and _pre5-r4 BUT Some more info after even more testing: -mpeg is not really a fix for the problem: I can more or less reliably fix the bug (at least for some time) by changing output-plugins on mplayer. It SEEMS that oss-output ist more prone to this bug than arts or alsa plugins. However it happens too with arts. I've not found out, what exactly triggers the reappearing of this bug. Sorry that I can give you so few infos :(
Can you please paste the output of: emerge -pv mplayer ? I'd like to see if there are any strange processor specific flags that are setting this off. Can you also try with -O2 instead of -O3?

Using CFLAGS="-march=pentium4 -O2" solves the problem for me.
