In Kmail 1.7 (kde 3.3.0), I select "Settings" "Configure Kmail" and I get an empty page, but when I press the  "help" button I get a KDE Help Center Window that says: 
"The requested help file could not be found. Check that you have installed the documentation."

The kmail handbook seems also to be out of date (version 1.5).

Yes. KDE needs you: http://kde.org/support/#Participating

I'm pretty sure it's known upstream that the kmail documentation is not up to date, but if there's not enough folks working on it, it doesn't happen (in time).
