Portage wants to install emacs-21.3-r4 in addition to the emacs-21.3-r3 that is already on my system, instead of updating to -r4 and unmerging -r3.

Sorry for the inconvenience. I fixed it in CVS. (moved SLOT was not 21 but 21.3)
It will be available in an hour. Thanks for the bug report.
