Changes:
- Recover from getting into a bad state when CCID becomes temporarily unavailable (which was causing problems on OS X Yosemite).
- Fixed modes being incorrectly reported under some circumstances.

Thanks for the report.  I have recently acquired a yubikey-neo but I honestly only bought it for testing so it's hard to me to keep up with their updates.  Please keep the bump request bugs coming :-)
