[    0.229707] uvesafb: failed to execute /sbin/v86d
[    0.229794] uvesafb: make sure that the v86d helper is installed and executable
[    0.229885] uvesafb: Getting VBE info block failed (eax=0x4f00, err=-26)
[    0.229969] uvesafb: vbe_init() failed with -22
[    0.233861] uvesafb: probe of uvesafb.0 failed with error -22

Re emerging both klibc and v86d made no difference. I am unable to reliably reproduce it, some times the boot process proceeds as expected. However, while the frequency of failures was very low under 3.15, under the new 3.16 it happens almost always. Prior kernels seem to be unaffected, but since it is random that is not 100% sure. It could be something else exposing an older bug unnoticed so far.

I do no longer have copies from when I was still using 3.15, this one is the current one. The issue remains and is still a random event.

Can you try adding USE flag x86emu (and rebuilding, of course) for v86d, and compiling uvesa as a module?

The x86emu was already enabled, it is profile masked. Compiling uvesa as a module made it like v86d was not even present at boot, no dmesg statement or the like. Attempting to manually load the module resulted in a black screen, even with the nvidia module disabled.

Also, after testing I upgraded to 3.17.1, same thing, it worked only twice so far, all other attempts were failures.

Can you try compiling uvesa as a module:
CONFIG_FB_UVESA=m

Can you also post your grub.cfg line? (assuming you are using grub).

Just tested setting it as a module under 3.18.1-gentoo. The results were the same as previously in comment #8.

Attached the newest dmesg under 1.18.2 I just upgraded to.

The relevant part is the same:
