I was noted a little strange behavior with MlDonkey 2.4.21. Just the files which are in current download are shared to upload. Other files into my shared folder aren't uploaded despite many requests. I found a thread into MlDonkey Forum where this issue is discussed and seems that there are some patches.

The bug is also present in MlDonkey 2.5.21-r1 (the core is 2.5-21c).

There is a patch for it (as well as other MlDonkey's bugs) available on this site:

Can somebody please add this patch to portage? 

I find MlDonkey the best ED2K client I've ever used only if it wasn't for the bug... :-(

As workaround you can use core-binary 2.5-21e, works fine.

The said patch is now in portage.
