qmail-spamc is not compiled / installed with Mail-SpamAssassin

Most people don't need qmail-spamc (that's why it's not build per default). Maybe a 'qmail' USE flag would make sense here. And IMO it should go to /usr/sbin, not /usr/bin.

No, there's no qmail USE flag yet. I'll try to get one ;-)

But the dependency on qmail is overkill IMO. qmail-spamc doesn't really need qmail, it's just a tool for qmail. And maintaining such a weird qmail dependency in the ebuild doesn't look fun.
