Installed Gentoo from "install-x86-universal-2004.0.iso" using provided stage3. I have a working system. Network is up and running, and proxies are specified. I started from the LiveCD snapshot. Following the Handbook, I first try to "emerge mirrorselect" to get the correct mirrors and then update Portage (using emerge sync).

In my case, emerge tries to download mirrorselect-0.82-r3.tbz2. But this file has disappeared from all the mirror mentioned in the mirror list. So "emerge" fails. From another machine, I went to see that actually the files are missing.

So I first ran "emerge sync" with the slow mirrors, then updated portage by itself (emerge --update portage") and I could finally grab mirrorselect with "emerge mirrorselect" which downloaded mirrorselect-0.83.tbz2.

Reproducible: Always
Steps to Reproduce:
1. Install from install-x86-universal-2004.0.iso
2. Use stage3
3. emerge --usepkg mirrorselect

Actual Results:  
mirrorselect-0.82-r3.tbz2 Not Found.

Expected Results:  
Should find the file and install it.

in this case the iso itself needs updating to 0.83, if the file is gone from the mirrors, however 2004.1 will be coming out soon with an updated mirrorselect, and the bug is very specific and theres an easy workaround, so this can be ignored (WONTFIX or something)
