I have mine stored in the ".icons" directory. I'm guessing that KDE isn't respecting the Xsettings - but you'll need to talk directly to the kdevelopers about that.
If I am not mistaken, the ~/.icons directory is where the themes themselves are stored. Quoting bugs.gentoo.org/whatisthis.html: Please note that bugs should be posted here first before moving upstream. In other words, don't bother the original package maintainers before you bother us. Our development team will move the necessary bugs upstream if needed.

The link you provided shows how to install cursor icons. This has nothing to do with setting the size of the cursor, only the images used. My problem is a bug that doesn't honor the settings in ~/.Xdefaults while KDE is running (other than during the splash-screen and logout).
