grsec.log reports:

grsec: denied RWX mprotect of /usr/lib/libavutil.so.51.35.100 by /usr/bin/mplayer[mplayer:1161] uid/euid:1000/1000 gid/egid:1000/1000, parent /bin/bash[bash:21594] uid/euid:1000/1000 gid/egid:1000/1000

disabling mprotect on mplayer make it working

if you have textrels or anything in media-video/ffmpeg you should report them instead; this doesnt seem related to mplayer at a first look

ffmpeg ebuild has this:
    if use pic ; then
        myconf="${myconf} --enable-pic"
        # disable asm code if PIC is required
        # as the provided asm decidedly is not PIC for x86.
        use x86 && myconf="${myconf} --disable-asm"
    fi

if you enable such grsecurity restrictions you should also enable the pic useflag, it'll never work otherwise.

Ok, then I'd suggest:

REQUIRED_USE="pax_kernel? (pic)"

Yes, but there isn't a pic IUSE.

no thanks; that'd just make the two useflags the same for no gain.
if you disallow non-pic libraries it should be obvious that you should build pic
