The forms the "Contributing Ebuilds" refers to have changed slightly.

You now have to select "New - expert" link at the bottom rather than just "New".

The initial entry on the bug page is entered into the system using the "Commit" button, not the "Submit" button.

Instead of "Attach a file to this bug" at the bottom of the page the form now says "Create a new attachment" about half way down.
