The problem is similar as described in the bug report under provided URL.

When I get disconnected (possibly because of connection limit time) modem disappears from networkmanager list (nm-applet, to be strict).  I have to reinsert USB modem to show up again.

Is this error still present with networkmanager-0.9.6.4?

If yes, then you probably want to help modemmanager developers diagnose the problem here:

I also share exactly the same experience. Some times unpluging and pluging the usb-stick device helps, and some time only after a restart it is recognized by nm again.

Also, upstream requests modemmanager log files (with more recent version) -> please jump to upstream report https://bugzilla.gnome.org/show_bug.cgi?id=671855
