Let's start with "before I fixed it yesterday it was failing to work in any situation" as it would abort due to fortified sources and/or hardened depending on the setup. Then we can continue with "upstream is dead, even the domain is gone", followed by "one of the patches applied actually breaks the logic of tftp to begin with". And finish off with "I tried seeing if there were other bad conditions and is the kind of code I don't want in by default".
Why not use the tftp from GNU inetutils? Latest update for that package (which we don't carry) is Jan of 2012, which is way better than netkit. Haven't tried it, though. Once I discovered how broken atftpd was w/ netbooting MIPS systems, I switched to netkit-tftp and it JustWorks(TM). Also discovered a mirror of netkit's old domain. Looks like there was a 0.18-pre1 devel release being worked on, circa 2002.
