I re-emerged media-libs/libsdl with USE="xv" support added, along with some other changes. After that was done, my "world update" succeeded, bringing media-libs/mediastreamer to 2.7.3-r3.
Please reopen. This is not a solution but a workaround. maintainer should enforce sdl or xv when video is selected.

If so, has this issue been resolved such that manual workarounds are no longer required in order to build media-libs/mediastreamer with xv support?
Thank you for report Willard! X video support requires xv to be enabled and thus it is not optional. I've dropped xv USE flag and forced to have it in all cases. Fixed. Jeroen sometimes your comments makes me wonder what have you meant by that?... :) I guess you've just mixed bug...
