got one question : which package still use it ? according to portage : grep -r "app-misc/mime-types" /usr/portage/*-* net-mail/sylpheed-claws net-www/webfs are the only 2 packages of portage using it. so, is it worth updating it ?
yes, it is worth updating :) if you write your own programs - e.g. python - you use this mapping. unfortunately the support for python is broken too: http://bugs.gentoo.org/show_bug.cgi?id=35284

The header block thing is a separate issue from the list being out of date. So, I filed a separate bug: bug 92638
