I'm experiencing this bug (same exact eerror) as well in ~amd64 with dev-db/mysql-init-scripts-2.0_pre1-r1. As a workaround, I've reverted to =dev-db/mysql-init-scripts-1.2. I am using the default init.d & conf.d files, so the issue is not being caused by a user modifying those files.
This is the line that's triggering it, but I can't see why. [ "${SVCNAME}" != mysql ] && rc_provide="!mysql"
Fixed in CVS. The conditional has a "failure" return value that was causing a false positive in detecting if the conf script was sourced properly.
