It seems to me xorg-server-1.7.7-r1 does not like "nv" driver.
When I booted I got blank screen. "X" was not running.

I downgraded to xorg-server-1.7.6 and "X" stars just fine.
I'm using "nVidia Corporation NV18 [GeForce4 MX 4000]" card

after upgrading xorg-server remember to rebuild your x11-drivers.
You can do this with the following command eix -I --only-names x11-divers/* | xargs emerge

The command you provided does not work:
eix -I --only-names x11-divers/* |xargs emerge

but I did:
emerge $(qlist -IC x11-drivers)

It rebuild 7 files:
x11-drivers/xf86-video-vesa-2.3.0

x11-drivers/xf86-video-nv-2.1.17
x11-drivers/xf86-video-fbdev-0.4.2

but after reboot I still get a blank screen, "X" does not start.

The X server attempts to load the nouveau driver. You can enable DRM_NOUVEAU in the kernel and build xf86-video-nouveau, or if you want to stay with nv you can create an xorg.conf containing only

Solved the problem, THANK YOU!

Would enable the "DRM_NOUVEAU" in the kernel and emerging "xf86-video-nouveau" driver enable second driver?  Which one will X use by default?

However, there is no such think in my kernel "DRM_NOUVEAU" or "NOUVEAU" I'm using kernel: linux-2.6.31-gentoo-r6
