I installed media-video/kffmpegthumbnailer-1.1.0, which pulled in media-video/ffmpegthumbnailer-2.0.2. I logged out, logged in, went to the dolphin preview preferences, unchecked the mplayerthumbs video preview and checked the kffmplegthumbnailer option The video thumbnailing works now.

So, shall we modify the dolphin[thumbnail] ebuild to pull in media-video/kffmpegthumbnailer instead? I reported a bug - https://bugs.kde.org/show_bug.cgi?id=244103 So we shall close this upstream.
For the users -- You can safely compile kde-base/dolphin with -thumbnail so as to remove the kde-base/mplayerthumbs dependency, then install media-video/kffmpegthumbnailer to get it recorded into the world favorite.
