X-Sane appears in GNOME's main menu 3 times:
1) Graphics -> X-Sane
2) Graphics -> XSane - Scanning
3) Sound & Video -> X-Sane

Bug about duplicate #3 already exists: it's 189458.
Duplicate #2 has not been reported yet, AFAIK. It's caused by installing 2 *.desktop files for xsane. One comes from xsane distribution, another is generated by ebuild with make_desktop_entry.

$ equery f xsane | grep desktop
/usr/share/applications/xsane-xsane.desktop
/usr/share/applications/xsane.desktop
