I need to configure my smtproutes to auth with the remote smtp server, because I
was unable to send mail to uol.com.br and hotmail.com. My machine uses a dynamic
dns and uol and hotmail do not accept mail from my machine. I plan to configure
my smtproutes to sendmail thought smtp.uol.com.br, but it requires auth. How I
include this patch in qmail? I try to include this manually but without sucess.

Try using the qmail-1.03-r12 ebuild.

The remote smtp authentication patch is included in the qregex-starttls-2way-auth patch found at: 
http://www.arda.homeunix.net/store/qmail/qregex-starttls-2way-auth.patch.

There's also a doc that describes how to configure it that'll be installed
in:
/usr/share/doc/qmail-1.03-r12/qmail-remote-auth-patch-doc.txt.gz

Why net-mail/qmail-1.03-r13 was marked as unstable? There are any know reason for that? What reason? Any specific problem that I shoud know? When it will be marked as stable?

standard policy, ebuilds are generally in unstable for a month before they are moved to stable. In the case of qmail -r13, that will be the end of this week.
