Every time a song is changed by pressing prev/next or double clicking on a song the volume of rhythmbox and the system is decreased. After about 3-4 changes like that the volume is muted and has to be fixed from within rhythmbox or next time it's muted again. Also the volume slider in rhythmbox doesn't correspond to the system's volume i.e. 16% in rhythmbox is 83% in the pulseaudio volume meter. Please report this upstream and paste the url here as this is most likely related to what's in the changelog about better support for flat volumes.
Had the same error (rhythmbox-0.12.3). It's gone for now. Maybe the update to libcanberra-0.14 helped? I've also restarted pulseaudio.
I can confirm that it has stopped reducing the volume after each song change for now.
Spoke too soon. I had downgraded to a previous version. I updated back again and it still lowers the volume.

If upgrading to 0.10.24 (when available) doesn't fix this, I'll take another look at it, but again, rhythmbox doesn't do anything with the volume on track changes, so it's hard to see what might need to be fixed there. </snap>
