I just noticed that qt-phonon didn't make any noise anymore. Turned out to be a missing gst-plugins-alsa on my system, which I seem to have purged using emerge --depclean some while ago. I think media-libs/gstreamer should respect the alsa USE flag and automatically emerge media-plugins/gst-plugins-alsa if set. I wonder why I had gst-plugins-alsa installed once, when it's not even a dependency of gstreamer?
Do you have gst-plugins-base installed? qt-phonon should have pulled in gst-plugins-base, which in turn should have pulled in gst-plugins-alsa if alsa is enabled (except for the currently masked version 0.10.21-r1, which instead builds the alsa plugin itself when alsa is enabled). Also, please post 'emerge --info' output.

I'm sorry, you may probably ignore this bug. I forgot that I'm using ~amd64 versions of all the gstreamer stuff.
