The ebuild has the options LINGUAS="-cs -fr -it -pl -ru" in portage selected on my machine yet for some reason when I start the game it is in Spanish. My system is set to en_GB locale so I can't see any reason why this would happen. What's even more odd is that "es" isn't even an option. Back when I emerged the game originally (years ago) it installed and ran in English so I'm not sure if something has been changed with the ebuild itself. Reproducible: Always Steps to Reproduce: 1. It's ID Software who messed up the default language, and obviously couldn't be bothered to fix it. Read pkg_postinst() in the quake4-bin ebuild. This is a CANTFIX, because it's a binary blob.
