I'm not able to reproduce it. With less than portage-2.1.3 it was normal for files to be left installed if their timestamps or md5 had changed at some point after they were installed. However, since portage-2.1.3, FEATURES=unmerge-orphans has been enabled by default. It ensures that files are always removed, except for those that are both protected by CONFIG_PROTECT and have been modified since install.
