The docs at the tail of a PHP emerge say to add the flag -D PHP to Apache, but 
I think this might instead need to be -D PHP4?

No, it's definetly '-D PHP' in all of the install to keep consistant. At one stage Apache1 and Apache2 said different things, and this was changed so that both said '-D PHP'.
