There is a new version of the iwl345-ucode available. It is version 2.14.1.5, which although is a lower version than the previous 2.14.4, it is newer. I will attach a working ebuild.

And the point will be? Noone will get the update with such broken versioning scheme.

Maybe remove the older one... or change the version... or add a note in the older one... or just add it for people like me who know this is newer and can mask the older one.

I assume that it is similar to the bump for ipw465, and that involves a break in interfaces, so wont bump this until the unstable tree becomes stable.
