agreed, upstream is very cooperative in working with good patches. Just ping the mplayer-dev ML about the purpose, etc. and they'll review it.
This patch was already reviewed once and nobody did the gruntwork needed to get it into shape (like factoring out the "lock - call function - while loop waiting for function to complete - unlock" that bloats the patch immensely). And if it is the same one as ubuntu applies it also hangs the Gui always and the non-gui whenever changing the volume quickly (at least I was shown the effect during LinuxTag). And last but not least, the current pulseaudio API will be replaced probably within this year by a new interface library and this one will be internal-only.
