The doc use flag is for "extra documentation (API, Javadoc, etc)". This is documentation that is outside the usual scope of user operation. Online help is part of the user experience and should always be accessible.

USE=doc has been used for docs that are huge and/or require huge dependencies as well, all over the tree. See the relevant debates on gentoo-dev mailing list. The gimp-help tarball itself is almost 42 megs to download, requires extensive dependencies to be installed, and takes quite some time to compile and install. Fix the mis-description of the flag, instead of forcing users to install unwanted stuff.
I agree to jakub, gimp-help is far too much overhead for being installed by default.
