I tried emerging sys-libs/pam-0.78-r5 and get the following error:

!!! A file is not listed in the Manifest: '/usr/portage/sys-libs/pam/files/pam.d-0.99/other'

After this, emerge quits.  I checked the Manifest file, and there is no mention of the other file listed for pam.d-0.99.  I have synced just prior to emerging.

I had to rm -rf /usr/portage/sys-libs/pam for it to update, and I now get this:
