After upgrading to XF86 4.2.99.4 from 4.2.1 the trident driver broke.
The symptoms:
The screen behaves as if the D-Sub cable was missing, except that the monitor
wakes up when starting X, so the driver does something to the screen. Xinerama
works and I can move the mouse to where the screen should be.

hi, I didn't think Trident was an Nvidia thing?  

seems like the nvidia driver is loading up -- is that supposed to happen?  If it is an nvidia, please remerge nvidia-kernel and nvidia-glx, if it is not, please emerge xfree-drm

The Nvidia geforce4 is my primary display (AGP) the rident is the secondary (PCI)

so neither of the displays are working?

The primary display is working fine, but the secondary (trident) behaves as if it was blanked, only it won't wake up.

Dunno what state it's in, but the monitor is active it just wont show an image.

Same problem with a SiS 6321 PCI and a S3 Savage4 PCI 
with the SiS I have the same problem as John and with the S3 my comp screen stays black and looks like it freezes. 
I got the following errors with the  SIS:

(EE) SIS(1): Cannot read V_BIOS
(EE) SIS(1): Cannot read V_BIOS 
(EE) SIS(1): Virtual height (0) is too small for the hardware (min 128)
(EE) SIS(1): xf86ValidateModes() error 

I think those last to are because of the V_BIOS

only thing that I found in the logs for the Savage4 was Cannot read V_BIOS...

Update on the situation:

I now have a third card (Matrox Millenium) which works fine, so it's not dualhead or pci related. Well fine... I have some problems with gnome-terminal on this display, if I use a background image and run f.ex. nano, background turns white and the ^X ^C and that stuff is the only visible on the screen, as black. Probably some error with the mga driver-gnome-ncurses stuff... hmm

The trident card however seems to be broken. I rebooted the computer with PCI as primary display. And some very strange behavior showed. I have a ASUS A7V333 mb wich has a splashscreen on boot. VGA seems to work as the bios manage to show the splashscreen. Next the POST proceeds but it skips the displaying of ide and ports (you know the square that show before MBR is loaded).

It also reports (has this funky speech POST reporter) that POST is complete and that the operatingsystem is beeing loaded (MBR), and then it hangs.
