qpkg documentation should contain instructions for updating the data base. On my system, after emerging the ntp pkg and updating to r4, doing qpkg ntp -c -v returned 
no output. After doing qpkg -I which did find ntp, qpkg did, infact, show expected output

qpkg is deprecated; whishes for the replacement tools (not named yet) will be solicited shortly.
