There is an ebuild for the PECL Translit package around, done by m@tacker.org. It needs reviewing and testing, and can then be included in the tree. It reportedly works with both PHP4 and PHP5 (need to test that).
Ebuild is attached as it was submitted (needs review still).

dev-php4/pecl-translit-0.5 and dev-php5/pecl-translit-0.5 are in the tree now.
