I've noticed that after upgrading to media-video/mplayer-bin-1.0.20060217 (and possibly X.org 7.0) mplayer doesn't work with xinerama anymore. This gives me the problem that the mplayer window gets way too wide because it fails to detect that I run a dual monitor setup. This happends with -vo gl, gl2 and xv, because x11 uses software rendering it doesn't have the same problem. The other ebuild (media-video/mplayer-bin-1.0_pre7-r3) does work correctly. If others have this problem, I forgot to tell that you can work around it by putting media-video/mplayer-bin-1.0.20060217 in /etc/portage/package.mask. This will mask this mplayer-bin build so the portage won't install this one anymore but up- or downgrade to another version.

I've noticed there's a new version of mplayer-bin (mplayer-bin-1.0.20060302) but it still has the same problem.
As noted by Rick, mplayer-bin-1.0.20060302 does not have xinerama support here either.. please recompile with xinerama flag.. This is almost become unusuable in it's current state.
This is still a problem in mplayer-bin-1.0.20060302, and it's completely unusable for a lot of people.
Fixed in 1.0.20060408.
