Just putting mirror://gentoo in SRC_URI should work fine. The restriction just means it doesn't check Gentoo mirrors prior to what's in SRC_URI.
My bad, Ciaran pointed out that my suggestion works with nomirror, not fetch.

Well, you could still do the mirror://gentoo thing for the patch and tell the user to manually fetch it as well. Might be worth checking whether portage will resolve the SRC_URI location to an actual URL.

In addition to large patches, will you address also the problem of too many patches in filesdir? Right now there are about 8300 patches in filesdir (about 20 megabyte of files). They are about 7% of the portage tree.
