While attempting to merge games-fps/quake4-demo-1.0, I get a Digest verification
failure:

Did you delete the file and try again?

By the way, emerge -pv quake4-demo does not list "cdinstall" as a possible use
flag. If "cdinstall" is active in make.conf, then the ebuild tries to install
quake4-bin and quake4-data as well.

I did delete the file. Portage redownloaded it from 
http://3dgamers.iinet.net.au/pub/3dgamers/games/quake4/quake4-linux-1.0-demo.x86.run 
 
However I still get the error.

USE=cdinstall does not do anything to quake4-demo ... you're mistaken about
whatever is happening on your box

as for the MD5sum, i just fetched from the URL you posted and got the same
result as whats in portage:
98ebbb835cc71ea29062cd2b4a139a46

can only assume your network/filesystem barfed
