Sometimes, it may be when I have a disk in that has the tab set to read-only
access, when I try to format, I will get the error "ioctl(FDFMTTRK):
Input/output error" when trying to format a floppy disk, subsequent attempts
give a device is read-only error, with no way to restore write access.

By the way, it even does this after I realized the tab was set to read-only and
changed it to read-write!

Ignore this bug, I suspect it is just my crappy hardware. :(
